We provide comfort through pain and symptom management including physical, emotional and spiritual needs of patient and their families during the final phase of terminal life.
We are a benefit program of health care and services designated to meet the special needs of patients and their loved ones, when the patient is diagnosed with a terminal illness for which they elect comfort and palliative care.
Our services are available for persons who are no longer able to benefit from curative treatment under a physician’s care. Elect to receive palliative and hospice care from our team of trained professionals to include our physicians, nurses, counselors, therapists, social worker, aides and volunteers whom will provide medical care and support to both patient, family and significant love ones.
Hospice Physician
• Collaborates with the Patient’s primary care Physician
• Meets with the Inter-Disciplinary Team
Hospice Nurse
• Skilled assessment of Patient’s palliative and pain management care
• Administration and management of Patient’s Plan of Care
• Oversees Inter-Disciplinary Team
Certified Home Health Aid / Hospice Aid
• Provide personal care for the Patient
• Including bathing, feeding, dental hygiene, dressing and physical fitness when necessary
Dietitian
• Available for consultation
Social Worker
• Assist with end of life preparation and or arrangements
• Provide emotional support
• Assist in finding community resources to help in financial situations
Chaplain
• Provide spiritual support to both Patient and their families
• Available to work with Patient’s own clergy
Volunteer
• Provide Patient with companionship, if necessary
• Helping to provide relief for the families to reduce stress during the final stage of their love ones passing.
Physical Therapist, OT, Speech Therapist, DME, Pharmacy Services are available upon request.
Bereavement Program, Inpatient Care, Continuous Care, Routine Care, Respite Care are also available upon request.
