The Hospice Care Benefit

Divine Hospice Care, Inc. accepts Medicare, MediCal, private insurance, and other forms of reimbursement for its services.

For care unrelated to the patient’s primary diagnosis, Medicare and Medi-Cal will continue to provide for their normal benefits.

Medicare and Medi-Cal programs offer coverage for Hospice services.

Since Divine Hospice Care, Inc. accepts Medicare and Medi-Cal as 100% coverage for its services, there are no out-of-pocket expenses for patients or their families to incur for the services related to their terminal diagnosis.

A Patient retains full Medicare coverage for any healthcare needs not related to the terminal diagnosis even when the patient elects hospice care.

In these instances, the Patient must continue to pay the applicable deductible and co-insurance amounts under the standard Medicare plan or the co-payment under the Medicare managed care (HMO) plan.

How Can You Start Receiving Hospice Care ?

Help is only one phone call away. The attending Physician, the Patient, a family member or significant friend, Clergy, a social worker, the hospital discharge planner, or any person sincerely concerned for the Patient’s well being can recommend Hospice Care.

We are here to assist you for the beginning of the Hospice program and the process of determining the Patient’s eligibility.

What is Hospice ?

Hospice is a special kind of palliative care providing comfort through pain and symptom management including physical, emotional, and spiritual needs of a patient during the final phase of their terminal illness.
Hospice is a benefit program of health care services designated to meet the special needs of the patients and their loved ones, when the patient is diagnosed with a terminal illness for which they elect to have palliative care, comfort, and dignity in their final passing.
Hospice services are available for persons who are no longer able to benefit from curative treatment under the direction of a Physician.
A team of trained professionals including Physicians, Nurses, counselors, clergy, therapists, social workers, aides, and volunteers will provide palliative care and non-palliative services to both the patient and their significant family members.

Hospice care can dramatically and positively impact quality of life. The goal is to empower the individual by providing knowledge and information to facilitate decision making. Our clinical staff are knowledgeable on pain and symptom management. They are committed to educate patients and their families and caregivers. Counseling and spiritual support are available to the patient and families.

Where Can Hospice Be Provided ?

Hospice care can be provided at home, at a skilled nursing facility, a residential care facility or wherever the patient chooses to reside.
